Job responsibilities include, but not limited to the following:

• Answer and respond to incoming calls, referrals, and requests for assistance
• Assess the needs of patients and families by phone, with special attention to the symptoms and comfort of the patient; provide immediate problem-solving support.
• Communicate significant findings, problems, patient’s needs, reports changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required.
• Maintain flow of communication between in-house staff, field staff and other departments, for coordination of activities and services.
• Provide Clinical knowledge to patients or caregivers questions
• Notify On-Call Nurse of patients inquiries and scheduling update\
• Update Continuous Care staff if necessary

Requirements:

• Current license as a Registered Nurse or LPN in Pennsylvania
• Acute care and/or home health experience is preferred.
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Must have a current drivers license
• Ability to travel to case assignments as assigned.
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills
