Job Title: LPN - Home Health Clinical Coordinator
Job Location: In office position East Stroudsburg
Shift(s): Monday - Friday 8:30am to 5:00pm
Traditional Home Health Care has an immediate opening for
LPN - Clinical Coordinator (Office-Based) (Mon-Fri)
Due to growth and expansion we are looking to hire in Monroe County
Full Time, Monday-Friday, Office Based LPN opportunity for our Home Health Office in East Stroudsburg, PA.
Position exists in Monroe County for LPN to perform clinical supervision of patients receiving home health services.
• Physician order follow-up
• Oversight of patient plans of care to ensure compliance with clinical interventions
• Schedules
• Phone Triage
This position will require some on call rotation.
We are looking for a candidate who demonstrates excellent organizational, communication, and customer service skills.
Home health experience required.
We pay highest rates, mileage reimbursement and competitive benefit package.
