Your Role (Varies by Discipline):

• Provide skilled, compassionate care to patients in their homes
• Collaborate with an interdisciplinary care team to support patient goals
• Complete accurate and timely documentation
• Educate patients and families to promote safety, independence, and confidence
• For On-Call RNs: Provide clinical support and guidance during assigned on-call hours

What You Need to Succeed:

• Active Pennsylvania License (RN, LPN, PT, OT, SLP, CNA as applicable)
• BLS Certification (for licensed clinical roles)
• Valid Driver’s License & Reliable Transportation
• Strong Communication & Documentation Skills
• Ability to Work Independently
• Home Health Experience (Preferred, Not Required)
