Job Title: RN/LPN Hospice Nurse
Shift: Full-time, part-time, and per diem schedules available. Our office hours are Monday thru Friday 8:30am to 5:00pm.
Job Location: New Milford, PA and surrounding areas.
Our nurses see patients in the most ideal setting - where the patient resides!
At Traditional, we do our best to keep each nurse's case load within a sensible geographic territory.

Job responsibilities include, but not limited to the following:

• Admit patients to Traditional Home Health Care (Hospice) and initiate comprehensive assessments. (RNs only)
• Develop and implement individualized plans of care in collaboration with physicians, nursing supervisors, patients, and family members.
• Provide direct patient care, including clinical treatments, personal care, medication administration, and patient/family education in accordance with physician orders.
• Continuously assess patient condition and respond to changes by consulting with physicians and adjusting the care plan as necessary.
• Maintain clear and consistent communication with physicians, nursing supervisors, therapists, and other members of the care team.
• Accurately and promptly document all patient interactions, care provided, medication changes, case conferences, and OASIS time points, as applicable.
• Act as a resource and advocate for patients by informing them of available services and making appropriate referrals to community resources.

• Current license as an RN or LPN in Pennsylvania
• Must have current driver's license
• Ability to travel to case assignments
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills
• Acute care and/or home health experience is preferred.
• Having experience with hospice patients or in the hospice environment is a plus.
• Understanding of Hospice philosophy and principles of death and dying.
