What You’ll Do:
- Provide one-on-one patient care in the home/school environment
- Implement and document care plans for quality and continuity
- Administer medications, treatments, and therapies
- Conduct patient assessments and coordinate care
- Report condition changes to the Clinical Manager and physician
- Educate and support patients and their families
