RN Case Manager
Due to expansion, and significant growth our Pediatric & Adult Skilled Home Care Division is seeking an aggressive candidate for a skies the limit opportunity. This position will be a salaried position paid weekly. Essential Job Functions/Responsibilities:
- Receives referrals, determines services required, and Agency’s ability to meet needs for pediatric and waiver patients.
- Supervisory visits to observe clinical skills and ensure exemplary client care and compliance
- Monitors documentation of pediatric and wavier patient clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
- Manages/supervises a team of RNs, LPNs, Home Health Aides to provide effective and quality home care services
- Current license as a Registered Nurse in Pennsylvania.
- Demonstrated organizational and time management skills, excellent communication skills, interpersonal skills, documentation skills critical thinking, and ability to work independently
- Ambition to grow and advance with company goals.
- Excellent communication and customer service skills
- Must be able to multi task in a fast paced environment
