Job Title: Licensed Practical Nurse-LPN
Job Location:  Clarks Summit, PA; Scott Township, PA and Nicholson, PA
Immediate Need:
Clarks Summit, PA - All SHIFTS AVAILABLE for a 24-HR a day case
Scott Township, PA - Monday through Friday 7:15am to 4:45pm 
Nicholson, PA - Monday through Friday 8:20am to 2:45pm
Rates: $28.00-$32.00

Job responsibilities include, but not limited to the following:

• One to One patient care in home
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
