Part Time/Per Diem Positions Available

Per diem positions exist for physical therapists to perform therapy visits to homebound patients on an intermittent basis. Patients' diagnoses include post ortho surgery, post long term inpatient stays, stroke rehab, general decline and fall risk patients.

Excellent visit rate and mileage reimbursement.

Duties and Responsibilities include, but not limited to the following:

• Evaluate visit and assess patient care and plans with patient/family and physician.
• Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence
• Teaches and supervises patient and family in the home environment regarding treatment, self-care, coping and preventive health measures.
• Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (i.e., social services, therapists, etc.) to carry out treatment plan objectives.

Requirements:

• Current license as a Physical/Occupational Therapist in Pennsylvania
• Must have a current drivers license
• Ability to travel to case assignments as assigned.
• Ability to work independently with minimal supervision and manage time effectively
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills
