Our 4.5 Star Agency is seeking a Full Time, Monday-Friday, "Office Based" Licensed Practical Nurse (LPN) opportunity for our "New" Home Health Office in E. Stroudsburg, PA. Position exists in Monroe County for LPN to perform clinical supervision of patients receiving home health services.

Duties will include:

• Physician order follow-up
• Oversight of patient plans of care to ensure compliance with clinical interventions
• Schedules
• Phone Triage
• Daily clinical operations oversight

*This position will require some on call rotation.
**We are looking for a candidate who demonstrates excellent organizational, communication, and customer service skills.
***Home health experience required.
