Registered Nurses (RNs) and Licensed Practical Nurses (LPNs)
To provide skilled nursing care to our consumer in their home located in Luzerne County

Location: Wilkes-Barre, PA
Monday-Friday:  8a-4p
Sunday-Friday:  9p-8a
 

Duties include, but not limited to the following:

• One to One patient care in home environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Qualification
• A valid Pennsylvania nursing license in good standing
• Current CPR certification
• Good organizational and communication skills
