Job Title: Music Therapist - Hospice
Job Location: Our Music Therapists see patients in the most ideal setting - the patient's home! 
Our office is located in Dunmore, PA 
Shift(s): Per-diem
Office hours Mon - Fri 8:30 - 5:00.

Duties and Responsibilities include, but not limited to the following:

• Evaluate, visit, and assess patient care and plans with patient/family and Hospice Supervisor
• Develop a music plan of care that is geared toward a comforting healing
• Work with the therapy team and use music as part of comprehensive therapeutic treatment plan
• Educate staff, treatment team, or other professionals regarding appropriate referral criteria for music therapy based on population needs
• Observe the client in music and or non-music settings
• Identify client functioning level, strengths and areas of needs such as, cognitive, communicative and emotional
• Identify clients clinical history, activity symptoms, cultural and spiritual background when indicated
• Must have integrity, a professional commitment to music therapy and the quality of services provided

Requirements:

• Must have a current drivers license
• Ability to travel to case assignments as assigned.
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills.
• Current certification as a Music Therapist Pennsylvania
