Job Title:CNA - Home Health
Job Location:East Stroudsburg Office - Monroe County and surrounding areas

Shift(s): Monday thru Friday 8:30 - 5:00p
Traditional Home Health Care is committed to providing the residents of the community we serve with quality home health care, while providing a rewarding experience for our care givers.
Job responsibilities include, but not limited to the following:
Thoughtful, caring and skilled CNA to assist patients on Traditional Home Health Care
Must have excellent training and experience with ADLs
Will be required to complete clinical documentation.
Assesses patient status and follows plan of care with coordination of nursing supervisor, patient and family members, and physician.
Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with MD orders.
Reports patient status changes within the electronic chart and communicates to RN Case Manager, nursing supervisor on a continuous basis.
Acts as a liaison and maintains contact with patients, therapist, nursing supervisor, and other members of the health care team.
Informs patients of other services that are available and makes referrals to other community resources as needed.
Requirements:
Current certification in Pennsylvania
Home health experience is preferred.
Excellent assessment skills, documentation skills and ability to meet deadlines
Must have a current drivers license
Ability to travel to case assignments as assigned.
Ability to work independently with minimal supervision and manage time effectively
Strong interpersonal skills.
Demonstrates good verbal and written communication, and organizational skills
