Job Title: Admin LPN Triage Nurse
Shift: Monday - Friday 8:30 - 5:00
Job Location: 113 W. Drinker St. Dunmore, PA 18512

Full-time position: LPN Triage nurse to take phone calls from Home Health and Hospice field staff as well as physicians. Responsible for call follow up, writing physician orders and communicating back to field staff. Triage LPN is a vital component in the timeliness of care for our patients. Excellent customer service and communication skills are required. 

Duties and Responsibilities:

• Answers phones in a professional and tactful manner
• Takes all necessary information on patients from field staff
• Ensures all medical orders are accurate and complete 
• Follow up on all staff and physician calls and orders
• Collaborates with clinical team to insure patient's needs are met
• Assists D.O.N. with Q.I. projects

• Current Pennsylvania LPN license
• Home health background preferred
• Computer / EMR savvy 
• Ability to work independently as well as collaboratively with team members and manage time effectively and efficiently
• Strong interpersonal and exceptional customer service skills
• Positive, proactive attitude
